On acceptance you will be invited to attend for a registration medical with a healthcare assistant. All new patients are asked to supply photographic ID and proof of address.

Underwood Health Centre has an established Complaints Policy and utilises the NHS complaints process 'Putting Things Right'. Should you have reason to complain, please ask to speak to the Practice Manager, who will provide you with the relevant Patient Information Sheet, and guide you through the Complaints Procedure.
